About CareForm
CareForm is the HIPAA-conscious form builder for small practices — plain-spoken, practice-manager-friendly. No jargon, no 'revolutionize'. We talk about the front desk, not 'the enterprise'.
We never give legal advice; we say 'check with your compliance counsel' when asked. We never claim 'HIPAA certified' — there is no such certification for software. What we offer: encrypted submissions, a BAA included at no extra charge, and DEMO mode that blocks PHI collection until the BAA is signed.
How this works. CareForm implements encryption at rest, role-based access, session timeouts, and an immutable audit log of every submission view and export. A BAA template ships in the product and is executed between CareForm and your practice at signup (have your counsel review it). No software is “HIPAA certified” — HHS does not certify products — and a BAA alone does not make a practice compliant; your own safeguards, training, and policies matter too. Forms cannot accept submissions until the practice's BAA is signed — enforced in code.
What we won't do
We don't do fake testimonials, invented customer counts, or guaranteed outcomes. Every claim on this site traces to something the software actually does — most of it demonstrable in the product repo today.
