The service

CareForm provides Form builder for small practices: encrypted submissions, role-based access, immutable audit log, healthcare templates. $29/mo — stop paying $129/mo for four forms. BAA template available at signup.

Billing

Plans are billed in advance via Stripe on the cadence shown on the pricing page. Cancel anytime from your Stripe customer portal; you keep access until the end of the billing period. Failed payments may pause the service after notice.

Acceptable use

Don't abuse the service, probe other customers' data, or use it for anything unlawful. We may suspend accounts that threaten the security or availability of the service.

Disclaimers

The service is provided "as is". We don't promise uninterrupted operation and we don't make outcome guarantees (no "never get fined", no "never get hacked", no "fully compliant" claims — those words don't appear in our marketing for a reason).

Regulatory honesty

CareForm implements encryption at rest, role-based access, session timeouts and an immutable audit log. A BAA template ships in the product and is executed between CareForm and your practice at signup; a form cannot accept submissions until that BAA is signed — enforced in code. HIPAA compliance is a property of how each party operates its safeguards, training and policies; no software is “HIPAA certified”, and HHS does not certify products.

Liability

To the maximum extent permitted by law, our liability is limited to the fees you paid in the 12 months before the claim.
